How to submit a claim

For quick processing of your claim ensure the following:

  • Send a detailed claim from your provider clearly stating :
    • Your membership number
    • Provider’s practice number and details
    • The claim must have the correct name of the person who received treatment
    • Your contact number
    • If you have already paid upfront you must attach a copy of your receipt for a refund.

  • Smartphone App: Send a photo of your claim via the Topmed Member App which can be downloaded from Google Play or the App Store
